I understand and agree that:
1. Medical Specialties of California UK Ltd does not guarantee that damage to the hair follicles or hair bulbs or scalp and alopecia will not occur when using the Penguin Cold Caps.
2. The Penguin Cold Caps are not approved by the Federal Drug Administration in the USA for scalp cooling to prevent damage to the hair follicles.
3. I indemnify the company Medical Specialties of California UK Ltd against any side effects and any damage created by using the Penguin Cold Caps.
4. I have consulted my doctor/oncologist regarding using Cold Cap Therapy with the Penguin Cold Caps and the detailed intended procedure has been fully explained to my oncologist and that he or she has given his or her approval and consent to use the Penguin Cold Caps for the purpose of scalp cooling to reduce chemotherapy induced alopecia.
5. I indemnify Medical Specialties of California UK Ltd of any claims consequential losses or damage etc. that may occur due to the use of the Penguin Cold Caps.
6. I confirm the caps will only be used by myself unless permission is given in writing by Medical Specialties of California.
7. On taking delivery of the Penguin Cold Caps I accept full responsibility for them and for any consequences that may arise.
8. The Law Governing this agreement shall be that of England in the United Kingdom.
